Growing evidence derived from cerebrospinal fluid (CSF), neuropathological, imaging, genetic, and epidemiological studies link neuroinflammation and immune dysregulation to a subset of individuals with a variety of severe mental disorders (SMDs), including affective and non-affective psychotic disorders. Further, the recent discoveries of neuronal surface antibodies (NSAs) in autoimmune encephalitis (AE) presenting with diverse neuropsychiatric disorders such as psychosis and cognitive decline, among many others, provides further support to the notion that CNS autoimmunity and neuroinflammation can contribute to the neurobiology of psychiatric disturbances. Further, these immune mechanisms m...
Adult and child psychiatric disorders, including mood disorders, psychosis, substance abuse, and dementia, represent a large source of disability in the general population. The urgency to develop new treatments that may have better efficacy and tolerance is explained by the fact that none of the treatments currently available can be considered completely satisfying. Several reasons underlie this relative failure, such as, for example, (1) lack of knowledge of the mechanisms involved, (2) heterogeneity of most psychiatric disorders (that have good inter-rater validity but weak relationship with specific neurobiological mechanisms), (3) variable tolerance that leads to poor compliance, and (4)...
After the original proposals of staging for psychotic disorders developed by McGorry and colleagues, a few systems have been put forward specifically for people with bipolar disorder. There is now consistent evidence that, at least for a significant portion of people with this disease, clinical course and outcome are not as benign as initially described. The evidence thus far points to relevant differences between early and late stages of bipolar disorders in the clinical course of illness, neurobiology and systemic pathology. These differences all suggest that staging is a viable addition to clinical care in bipolar disorder. Neuroprogression and Staging in Bipolar Disorder provides a compr...
