In this Special Issue, we present a conceptual framework which provides a blueprint for the components of successful national organ donation and transplant programs, and can be used for their assessment. Applying this framework, we then present a series of country case studies that highlight the distinctive features that have contributed to the success of the organ donation and transplant programs of Croatia, Italy, Portugal, Spain, and the United Kingdom and elaborate on the lessons that can be learned from them. The Transplantation Learning Journey (TLJ 3.0) is the closing event of a one year leading international process, aimed at creating consensus among experts on burning transplant issues incompletely covered by scientific evidence. Through the TLJ 3.0 consensus platform, the European Society for Organ Transplantation (ESOT) demonstrates a strong commitment to providing clinical guidance based on rigorous evidence analysis and expert discussion within a methodologically sound and transparent process. This Special Issue compiles the guidelines from this process, which represents the first step of a continuous, regularly updated consensus program that ESOT envisions to shape future clinical pathways in increasingly complex transplant scenarios.
Organ transplantation is a life-saving surgical procedure through which the functionality of a failing organ system can be restored. However, without the life-long administration of immunosuppressive drugs, the recipient’s immune system will launch a massive immune attack that will ultimately destroy the graft. In 2018, the European Society for Organ Transplantation (ESOT) initiated a Broad Scientific Advice request to the European Medicines Agency (EMA), to discuss their Committee for Medicinal Products for Human Use (CHMP) guidelines relating to the development of innovative therapies in the field of kidney transplantation. The request suggested several options for redefining endpoints used in clinical studies of kidney transplantation. It also proposed study design elements for stratification and subsequent employment of these novel proposals into future clinical trials. The proposed endpoints also aimed to be suitable for the conditional marketing authorization pathway. The EMA subsequently evaluated the discussion document and evidence provided by ESOT, issuing their final recommendations in December 2020. This Special Issue of Transplant International provides a detailed summary of key aspects of the proposals and recommendations for definitions of endpoints for clinical trials of kidney transplantation.
